How Body composition / DEXA works – and what the evidence says

DEXA (dual-energy X-ray absorptiometry) measures regional fat mass, lean mass and bone density with a very low radiation dose – precise enough to detect changes of a few hundred grams. InBody and other bioimpedance devices estimate composition via electrical resistance: quick and cheap, but more susceptible to hydration fluctuations. 3D scanners capture circumferences and posture optically.

VO2max is determined by spiroergometry on an ergometer or treadmill: breathing gases are measured up to exhaustion. Large cohort studies show that high cardiorespiratory fitness goes hand in hand with markedly lower mortality – more strongly than classic risk factors.

A baseline plus follow-up measurements every 6 to 12 months makes sense.

Maximum oxygen uptake is a measure of cardiorespiratory fitness. In large studies, people with high VO2max have a markedly lower risk of death – the effect is larger than that of smoking, diabetes or high blood pressure. Good news: the value is trainable.

What happens in a VO2max test?

On a cycle ergometer or treadmill you increase the load step by step to exhaustion while a mask measures your breathing gases. The test takes 45 to 75 minutes including preparation and provides training zones as well as VO2max.

Is the radiation from a DEXA scan a concern?

The dose of a whole-body DEXA is far below that of a transatlantic flight. Pregnant women should nevertheless abstain.
